Orthostatic tremor: Two persons in a balance disorders practice

ABSTRACT

The objective of this paper is to help physical therapists recognize orthostatic tremor. sum of two units cases of orthostatic tremor are reported from a tertiary referral center The 2 patients not awayed to the Balance Disorders Clinic with complaints of imbalance and shaking in the leg while standing on the contrary not with walking. Postural sway was recorded during the Sensory Organization ordeal of computerized dynamic posturography and was analyzed for shear and center-of-pressure composings Power in the shear force representation and the center of squeezing power measures were increased compared to 18 normal superintendences in both patients. Patient 1 underwent balance rehabilitation and improved a certain number of with a reduction in the power in his shear force representations during dynamic posturography. He was then treated with gabapcntin. Patient 2 was provided gabapentin and later was sent to physical therapy. A reduction in the power of the shear force representation also was noted. Using shear force data from computerized dynamic posturography to assist in making the diagnosis of orthostatic tremor has not previously been reported. In addition to medication, physical therapy may be an effective adjunct in individuals with orthostatic tremor.

INTRODUCTION

Orthostatic tremor is a terminus that describes a rare tremor that is pronounced in standing and is les stiff in sitting or walking.1-5 Pazzageli et al5 first described the condition in a manuscript written in Spanish in 1970 The first one in the English literature to describe and coin the mete orthostatic tremor was Heilman.1 Heilman described 3 cases in 1984 and above 100 cases have since been documented in the literature.1-39 Because of its rarity, orthostatic tremor occasionally has been mislabeled as a psychiatric disorder.2,39 Orthostatic tremor affects slightly more women than men152122 and is usually noticed in middle aged to older adults.2-4,6-8 Patients may have had symptoms for greater than 20 years before seeking attention.2

Orthostatic tremor is characterized by the agency of unsteadiness that occurs within next to the firsts of standing, but almost always within 1 minute (Table 1)346 Symptoms are typically relieved through sitting, leaning while standing, walking, or lying supine.9,10 Rocking back and forth in standing can cause the tremor to be les obvious to the patient, although it is still present11 Although the symptoms of orthostatic tremor are relieved during gait, Hanna et al12 and McManis and Sharbrough6 have demonstrated that the tremor actually persists during ambulation on the contrary is not noticed by the patient.

The boundary orthostatic tremor is a misnomer as the condition actually arises in various positions.11 Walker hints that it should be renamed 'isometric tremor' or 'weight bearing tremor.11 Orthostatic tremor has been seen in the upper extremity with the patient positioned upon hands and knees,3 in the supine position during isometric contractions,8,11,13 and when a patient was lifted not upon the ground with a suspension a whole and performed an isometric contraction.9 When suspended overhead with the lower extremity muscles relaxed, no tremor was noted.13

Recently it has been allude toed by Yarrow et al40 that somebodys with orthostatic tremor have an increase in the change of the center of crushing (COP) at the higher frequencies during quiet standing. We report upon 2 patients with orthostatic tremor including originates of the Sensory Organization trial of computerized dynamic posturography. We observ that each of the 2 patients with orthostatic tremor had unusual shear forces during quiet standing one as well as the other with their eyes open and clos frequent occurrence spectra of shear-force and COP recordings for the one and the other the eyes open (EO) and organ of visions closed (EC) conditions were calculated in Matlab (version 512 The Math Works, Inc., Natick, Mass) using Welch's averaged periodogram course [with 1000 data points (20 sec @ 50Hz) and a 256-point Manning window] Total spectral power, as well as power in the 0-5Hz and 5-25Hz frequent occurrence bands (expressed as a percentage of total power) were compared among 18 age matched superintendence subjects whose data existed from other casts and the 2 persons with orthostatic tremor. All of the 'control' controls had a normal neurologic examination as performed by means of a board certified neurologist.

CASE ONE

History

A 70-year-old man not awayed with complaints of unsteadiness for the past 7 years while standing. Mr J could alone stand for 2 to 3 minutes before he felt real unsteady and needed to lean against something for support. He reported that he felt that he might fall forward or backward. Mr J felt that his symptoms, which began 6 years prior to being seen in physical therapy were getting progressively worse. He reported having great difficulty with standing at parties and also of having difficulty with putting upon the golf course. The patient had bilateral hearing los and arthritis. His past surgical history included resection of a colon cancer and prostate surgery The patient was taking simvastatin (Zocor). He was consuming approximately 6 ounce of alcohol and 3 bowls of coffee per day.
