Surrendra Patel worked as a "set-up" man for Concord Industries Inc.,
in Franklin Park, Ill. His piece of work required him to change the mold and
dies upon presses. On Sept. 4, 1985 single of the trim presses was not
cutting fitly and Patel tried to fix it. Because the power disconnect
switch was not working, Patel purely pressed the stop button upon the
front of the machine. Patel had his hands in the machine when a coworker
apparently crushed the start switch by accident. The machine activated,
cutting not upon Patel's fingers.
