Content Validation of Parental Role Conflict in the Neonatal Intensive Care Unit

PURPOSE To validate the easy in mind of the nursing diagnosis parental character conflict made by neonatal intensive care (NICU) nourishs and researchers in the field and to identify the related major and minor defining characteristics.

METHOD Fehring's Diagnostic contented Validation model using 59 neonatal specialists.

FINDINGS. Nineteen defining characteristics were validated for use in NICUs: four were considered major and 15 as minor. There was no correlation between nurture profiles and defining characteristic scores.

CONCLUSIONS. In this inquiry parental role conflict was validated for use in neonatal units on the contrary defining characteristics must be described in a clearer and more objective manner.

NURSING IMPLICATIONS. Validation studies in different healthcare settings are required in the way that that the specificities of each clientele are met NANDA diagnoses must be proofed and validated at neonatal units, because this setting has discloseed few studies and their use in practice is a potential way to improve nursing care.

The idealized maternal part anticipated by the mother is changed when her newborn is hospitalized and she becomes hanging on the medical and nursing team to obtain information about and make contact with her baby. Unfortunately, the relationship with the healthcare team may become another source of distress when there are discrepancies between expectations and perceptions (Fenwick, Barclay, & Schmied, 2001) Furthermore, the mother has to deal with regards about the infant's survival chances, potential sequelae, and the impact of the birth upon the family (Fenwick et al., 2001; Miles, Carlson, & offensive smell 1996; Miles, Holditch-Davis, Burchinal, & Nelson 1999; Wereszczak, Miles, & Holditch-Davis, 1997)

In the neonatal unit, mothers are encouraged to gradually take above the task of caring for their children. Studies have described maternal feelings during the hospitalization period: depressed self-esteem, failure, and inability to get the upper hand of the difficulties resulting from the experience of caring for someone (Costello & Chapman, 1998; Fenwick et al., 2001; Tronchin, 2003) Mothers verbalize their difficulties and feelings of insecurity in taking care of their newborn and in recognizing and solving question at issues

These events can be identified and interventions can be propos from one side the use of nursing diagnoses. Parental part conflict is one of the diagnoses that ne to be researched with regard to mothers of babies in neonatal intensive care units (NICU). The definition of this diagnosis according to NANDA International (2003) is "parent experience of character confusion and conflict in replication to crisis" (p. 37). Hospitalization of the baby provided the adjoining matter for a crisis for this close attention

The maternal role is that performance focused upon providing an environment favorable to physical expansion and development of the baby (Carpenito, 2002; Fenwick et al., 2001) Thus, the NICU is a favorable environment for close attention of parental role conflict because hospitalization of a newborn means, to the mother, many modifications in the character they had expected to play. They are dealing with many negative feelings like as fear, separation from their babies, and los of the "perfect" child (Klaus & Kennell 1992)

Caring for the baby should imply caring for the mother as well; thus, maternal nursing diagnoses also should be made in NICUs. Offering better conditions for the mother to care for her have a title to child and maternal care can support to the baby's recuperation and help ensure future care extremitys are met. Lewis-Abney and Rosenkranz (1994) and Carpenito (2002) have emphasized that the ability of nursing diagnoses to direct interventions and their impact upon the patient are directly related to the amplitude to which the existing nomenclature has been critically evaluated and systematically proofed Therefore, the parental (maternal) part conflict should be submitted to trials and refining for the establishment of more appropriate interventions.

According to Carpenito (1998) it is still a challenge to have nurtures value and apply nursing diagnoses in health care. This is likely related to the healthcare pattern centered on medical diagnoses and not upon patient response to the facts failure of undergraduate courses to integrate nursing diagnoses into the curriculum, and lack of perception of nurtures who actually propose diagnoses all the time on the other hand do not write them in a standard manner.

Purpose

The final causes of this study were to validate the contented of parental role conflict taking into account the mother, using nourishs who work in NICUs or who have carried on the outside studies or have a specific education upon nursing diagnoses related to this area and to identify the major and minor defining characteristics of parental character conflict related to mothers of newborns being cared for in a NICU.

Method

This descriptive inquiry was used Fehring's Diagnostic contented Validation model (1987). Nurses, prefered according to the criteria of this validation design were used to determine the step to which certain defining characteristics were indicative of the diagnosis (Fehring, 1987; Garcia, 1998)
